We need the bill for treatment, the reason (illness, accident or maternity) with a few details of the treatment and proof of payment. If you were involved in an accident, an accident report is also required.
- Language: The bill must be in English, German, French or Italian.
- Services: An exact list of the treatments is required.
- Insured person: Name and insurance number must be stated on the bill.
